Thank you for coming to the Parent Portal.

NOTE: You must have your Student Athletes Student ID Number to complete the forms.

1. To access the required online forms click on the “Electronic Participation Forms” tab.

2. Click on the form name, read and complete the information requested.

3. Only High School is required to fill out the "TAPPS Acknowledgment of Rules” form

4. To sign the documents, click inside the signature box and hold your mouse over the box. This will allow you to create an electronic signature.

5. Upon completion of each document and saving, you will have the opportunity to save for your records also.

6. Once all of the electronic forms are completed, move your cursor over to the “Download and Print” tab and click. Here is where you will download and print the Physical form to take to your physician.

7. Once your physician has completed the Physical Form, please scan and email the form to [email protected]
